Complete Intake Form
Download and complete the Peptide Purity Testing Intake Form. This form provides the information needed to correctly process your submission and generate your Certificate of Analysis.
Important: The Company / Organization Name listed on the intake form will be the only name displayed on the Certificate of Analysis (COA).

Authorize Payment
Payment authorization is required before testing begins.
Secure Online Credit Card Authorization (Recommended)
Submit your card information through our secure payment portal. Card information is encrypted and transmitted directly to our payment processor. Freedom Diagnostics does not store full credit card numbers.
Your card will only be charged after samples are received and testing begins.

Prepare Your Samples
- Ensure vials are securely sealed and protected during shipping.
- Clearly label each vial.
- Confirm lot numbers match the intake form.
- If submitting multiple vials for batch conformity testing, ensure they are from the same lot.

Please include the following in your shipment:
- Completed Intake Form
- Payment Authorization
- Properly labeled sample vials

Processing Timeline
